Drug & Alcohol Evaluation Certification
Issued on completion of the $165 flat-rate evaluation. Confirms the clinical interview was conducted, the screening instruments were administered, and a signed diagnostic report was produced.

At intake, you name the attorney, probation officer, or court of record to receive the certificate.

Certificate is printed on watermarked stock, embossed with the clinic seal, and signed in ink.

Original mailed, PDF emailed — both within 72 hours of completion at no additional fee.
We verify
Recipients can call the clinic to verify the certificate number against our internal register.
