Every evaluation is signed by a licensed behavioral-health clinician and includes the language courts, probation officers, and defense attorneys look for. No generic "recommendations" — an ASAM level-of-care recommendation, a diagnostic impression in DSM-5-TR language, and the screening scores that support both.

Court, attorney, PO, employer, or self-referral — documented.
Clinical interview
Substance-use history, mental-health history, social history, legal history.
Screening scores
SASSI-4, AUDIT, DAST raw scores + interpretation.
Collateral review
Prior assessments, police reports, court docs you provide.
Diagnostic impression
DSM-5-TR language — or explicit "no substance-use disorder diagnosis."
ASAM recommendation
Level-of-care recommendation tied to Criteria 4th Edition dimensions.
Signature + credential
Licensed clinician, license type + number, date signed.
